Inclusive page number(s) of the article.  

Title of the chart/table/figure (e.g. Table 1, Estimate UK prevalence of diabetes);

Page number for the chart/table/figure.

If you have adapted the image/chart/table in anyway, add [adapted] in square brackets at the end of the reference.
