Indicate the court in which the case was heard in brackets after the first page of the report, and before the pinpoint if there is one.

Example of case without neutral citation but with a pinpoint to specific paragraphs:
R v William Clive Roberts [1999] 1 Cr App R(S) 381 (CA) [8]-[15].
Example of case with a neutral citation and pinpointing:
Roberts v Gable [2006] EWHC 1025 (QB), [2006] EMLR 23, 26
The only differences are that case names are not italicised, pinpointing is not used and there is no full stop at the end of the reference.
